VETERINARY REFERRAL FORM
Letter of Introduction

To Whom It May Concern:
Your client is seeking a referral for my services, I am a Certified Professional Canine Fitness Trainer (CPCFT). This letter is intended to clarify my qualifications, my role in your patient’s care, and my general goals for dogs seeking my services. I would like to ensure you approve of your patient working with me. Please advise me of any diagnosed acute or chronic injury or any other health concerns or restrictions.
The overall goal of canine fitness is to improve strength, flexibility, coordination, and overall quality of life. I work with dogs of all breeds, ages, and skill levels through individual conditioning programs and group canine fitness classes. The certification process to become a CPCFT includes: 24 weeks of online classes, four 6-8 week comprehensive peer-reviewed case studies, and passing a final exam. The process takes approximately 12-18 months to complete. This program is overseen, developed, and taught, by Dr. Deborah Torraca, DPT, MSPT, Diplomat ABPTS, CCRP through the Fenzi Dog Sports Academy. Class instruction includes basic anatomy, conformation and structure, gait analysis, proper form, safety, nutrition, ethics, and scope of service.
During my studies I have continued to be involved in the Canine Fitness community being active in CPCFT discussion groups, and taking additional Canine Conditioning courses. I have worked with a variety of dogs such as: puppies, seniors, pet dogs, and performance dogs. Conditioning programs are customized for the needs of each dog and the owner’s abilities. Program design is conservative and focused, building a strong foundation prior to introducing complex or advanced exercises. Proper form and safety is the upmost priority. My goals are to provide canine fitness and conditioning services which are complementary to regular veterinary care and/or continued strengthening after completing rehabilitative programs. When concerns are noted, clients will be directed to seek appropriate veterinary care prior to continuing to work with me.
I am also a Certified Cold Laser Therapy Practioner. I studied with the Holistic Animal Studies, with Dr. Angelique Barbara, Doctor of Chiropractic who holds additional degrees in Veterinary Science (B.S.), Equine Science (Minor) and Veterinary Pathobiology (M.S.). The certification process took 26 weeks of online classes, an acupoint laser therapy video case study, 6-8 week comprehensive writen case study, and passing a final exam.
Laser stands for Light Amplification by Stimulated Emission of Radiation. Laser uses light energy to affect the tissues targeted. Laser therapy essentially, provides energy to the body, causing bodily processes to occur. This effect is called photobiomodulation. I use a class 3b laser. This type of laser is also referred to as low level laser therapy or cold laser. Cold Laser therapy has many benefits such as to accelerate soft tissue repair, anti-inflammation and relieve pain. It can effectively help treat the following conditions;
Joint Injuries
Nerve Injury
Chronic Pain
Ligament or tendon injuries
Skin lesions or abrasions
Post-trauma wounds
Post-surgical wounds
Anti-inflammation
Wound healing
Hematoma
Muscle sprains or strains
Bone arthritis / Fracture / Dysplasia
Trigger Points
Ear Infection
Abscessed Tooth /Gingivitis /TMJ Pain
